Dear Parents/Guardians:

Houston County Public Health is working with your child’s school to give the annual influenza vaccine to children at school.  We will have available quadrivalent injectable vaccine, which offers protection against four strains.  We will hold vaccinations clinics again this fall.  Pre-registration will be required, along with providing a copy of a valid insurance card. (Blue Cross of Minnesota, Blue Plus and/or UCare). Please note that all Blue Cross numbers have changed this year.

We can also bill Blue Cross/Blue Shield of Minnesota---for both children and adults.  The cost for students and adults who do not have BC/BS of Minnesota is $40. 

We will be offering flu clinics on:  Wednesday, October 4th from 2:30 -4:30 pm. You must pre-register no later than September 27th. If you are not pre-registered for this clinic, you are welcome to schedule an appointment at our office in Caledonia.

Parents must accompany elementary students from K through 3rd grade to the clinic.  If your child is older and is able to attend the clinic without a parent, they must bring a signed consent form along with a copy of the accepted form of insurance coverage or cash.  You can find the consent form is on the "Forms" button to the left and can be downloaded for your convenience.
